How to I register as a new user?

Create an account with an email/password or single sign-on service.

What does the registration process involve?

1. There are two ways you can register for a new account:
   1. Sign up with a preferred email and choose a unique password
   2. Use a single sign-on service by selecting the Google or Apple account icons.
2. Verifying your account could mean obtaining a security code in your email, entering an Apple passcode, or logging in to your Gmail account.
3. Registration requires

   1. Full Name

   2. Date of Birth (DOB)*

      *Only those 13 and older are allowed to register an account on Primary Record

When registering an account with Primary Record, you can explore the application by creating one free Medical Profile for yourself or someone else.  You can subscribe to a Family plan to add more Medical Profiles.

Know when this subscription plan is paused or canceled, the Medical profiles you created and all the medical data organized are still in your Primary Record to view, edit, or add to manually.

Inviting Others to Help You is in Your Control

When you create a Medical Profile for yourself or someone else, you become the Profile Organizer, with the ability to control sharing permissions under the 'Manage' section of a Medical Profile. Additionally, you can initiate a transfer to another Collaborator if you want them to take on the role of Profile Organizer.
