Why do families need to transfer a Medical Profile?
Transferring a Medical Profile allows Profile Organizers to pass profiles to a new owner during important family changes. Whether you have a teen transitioning into adulthood, an aging parent who now requires guardianship, or a foster family returning a medically complex child to their parents, there is a growing need to make sure the Medical Profile stays with the person best able to act as the Profile Organizer.

What are the steps to transfer a Medical Profile to another Profile Organizer?
- Select the Medical Profile you would like to transfer.
- Review the Manage tab to make sure the person you are transferring is already a designated Collaborator.
- If they are not, invite the future Profile Organizer as a Collaborator first.
- Once accepted or confirmed, select the three vertical dots in the top right-hand corner of the Medical Profile.
- Use the drop-down menu to select the Collaborate you are transferring.
- Select “I’d like to maintain access” if you would like to request the future Profile Organizer to consider you as a Colalborator upon transfer.
- Send Transfer Request
